2. Print Out the Order Form.
3. If you don't have a copy of your most recent prescription from your doctor, Print Out the Prescription Form here. Have your doctor fill it out. Either you or your doctor can FAX it back to us.
4. FAX Order Form and Prescription (or Prescription Form) to Chase Ergonomics at (505 ) 344-1426. Be certain to include an email address, FAX or phone number so we can confirm your order. Your information will not be shared with anyone else.
